RISK ASSESSMENT ACTIVITY REPORT

Task

Risk Assessment Form

- Identifying, in the left hand column, at least 8 hazards, with a range across at least four topics from the syllabus eg; fire, manual handling, mechanical and electricity…etc
- Identifying in the next column who might be harmed by each hazard and how they will be harmed.
- Identifying in the next column what is already in place to control the risk.
- In the next column stating ‘YES’ if further controls are needed or ‘NO’ if existing controls adequately control the identified hazard (you need 3 control measures).

RISK ASSESSMENT ACTIVITY REPORT

- Identifying in the next column any additional control measures necessary to reduce the risk presented by each hazard (if you have 2 control measures in column 3, you should say YES and add one control measure in column 5)
- In the final column prioritizing proposed control measures using a numerical scale (5 as high priority (for high risk) and 1 low priority (low risk).

Hazard identification

Good Examples on specific hazard:

1. Prolonged exposure to the loud noise produced from steel production machinery.

1. Overturning hazard occurs as a semi-truck is used to transport huge steel beam

3.   Exposure to wood dust produced by grinding machines

Who might be harmed and how

For the ‘who’ criteria, you should give thought to who might be harmed (groups of people likely to be affected e g visitors, staff, employees working in the area etc.)

For the “how” criteria, how they are likely to be harmed e.g. broken bones, asthma etc

NO Half mark if you state WHO only or HOW only Good Examples on Who might be harmed and how :

- Workers who are doing the job. Fatigue and stress might occur with the passage of time in addition to hearing damage.
- Workers who are carrying out the job. Their skin may come into contact with the hazardous substance and absorb it which cause living tissues to be destroyed.
- Workers and visitors may suffer temporary or permanent hearing damage from exposure to noise from drilling machines.

What’s already being done to control the risk? / additional controls

In this section, you should compare the existing controls with good practice and state either ‘yes’ or ‘no’ in the “any further controls” box.

If you decide no additional controls are needed, you should explain why no additional actions are required in the “additional” box.

Good Examples on What is already being done to control the risk?

- Workers are wearing ear plugs to reduce the effect of the noise.
- Inspections are done regularly to ensure use of PPE (Ear Plugs).

Additional actions

- Health surveillance should be monitored to ensure having effective control measures.
